COVID-19 cough survey

This is a survey to help us develop algorithms that detect COVID-19 from voice and cough sounds.

You will be asked to record and submit short voice/cough samples and respond to a series of questions about your demographics and health status, such as whether you've been tested for or diagnosed with COVID-19.

You can find more information about the project in this Information Sheet.

Please note that this site is not compatible with non-Safari browsers on iOS devices or Internet Explorer. Please report any issues to covid@novoic.com

To proceed with the survey, you (or the person you're filling this out on behalf of) must agree to the statements below. Check the box next to each statement to indicate your agreement and then click 'Next'.

Can you tell us the following basic information?

We will now ask some questions that will help us evaluate %pp% symptoms.

We'll now ask some questions about things that are not COVID-19, but might affect %pp% coughs.
